ABSTRACT Colposcopy has become an essential tool for clinical diagnosis of cervical carcinoma. Despite the fact that cervical adenocarcinoma accounts for approximately 10% of total cervical carcinomas and has recently been increasing in number, there have been very few reports on its colposcopic appearances. We have classified colposcopic features of invasive endocervical adenocarcinoma into five different patterns: papillary, transformation zone-like, atypical vessels, nodular and net-like findings. Based on this criteria, colposcopic diagnosis is possible not only for frank invasive but also for early invasive cervical adenocarcinoma with 3 to 5 mm invasion in depth, but impossible for adenocarcinoma in situ or microinvasive adenocarcinoma. These observations may be quite useful for colposcopic diagnosis of glandular abnormalities of the uterine cervix.
