Comparison of serum IGRA test with PPD test for diagnosis of tuberculosis (TB) in HIV-positive patients

ABSTRACT
 
In spite of the many efforts to control tuberculosis in recent years, with an increase in the number of people infected with HIV, its prevalence is increasing in these people. The use of purified protein derivate (PPD) is a common diagnostic method of tuberculosis, but cannot be considered as a standard method due to false positive and negative results. Interferon-gamma (IFNγ) is also a new test. The purpose of this study is to compare these two diagnostic tests in identifying patients with tuberculosis in HIV-positive people. 50 patients with HIV were selected from our Center. All the HIV-positive patients were included in the study and patients on anti-tuberculosis treatment were excluded from the study. Traditional tuberculin skin test-purified protein derivative (TST-PPD) test with a test period of 48 h was performed by intradermal PPD injection. Quantiferon test was performed on patients’ serum. Data were analyzed using SPSS software by T-test, Chi-Square and Kappa criteria for matching the two tests. 26 patients (52%) were positive for the PPD test and 24 patients (48%) were negative. 20 patients (40%) were positive for the Quantiferon test and 30 patients (60%) had a negative result. After comparing the tests, the agreement rate for identifying positive cases between the two tests was 42.3% whereas the negative result was 62.5%. The overall agreement rate between results of the two tests was estimated to be 52%. The agreement rate between the tests in Kappa test was κ = 0.68, indicating the similarity of the results of the two tests
